Liability Waiver

I hereby acknowledge that I have volunteered to participate in a paid or unpaid coaching, training or spiritual counseling, to include, but not limited to, any and all services provided, such as an reiki, meditation, advice, guidance, and or product offerings. I understand that my work with Kelsey J Patel is a complimentary service and healing modality that does not substitute for medical, or therapeutic advice, and it is my responsibility to secure such advisement. I acknowledge and agree that no warranties or representations have been made to me regarding the results I will achieve from any insight, guidance, product, or counseling. I understand that the results are individual and may vary.

I do here and forever hold Kelsey J Patel Inc. and all subsidiaries harmless from any possible effects that may cause temporary physical or emotional discomfort. I do here and forever release, discharge and hereby hold harmless Kelsey J Patel Inc. and her respective agents, heirs, interns, and employees from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with my participation in a reading, purchase, counseling, or program without limitation, which may occur as a result of following advice tendered and released, teaching rendered, or the use of facilities during a session or event.

 

I further acknowledge that I will not record my reading, training and/or session with Kelsey J Patel. If provided with access to recordings of readings/trainings and/or sessions, I acknowledge that distribution or sales of that recording are strictly prohibited by unauthorized parties. 

 

I ACKNOWLEDGE THAT I HAVE THOROUGHLY READ THROUGH THIS WAIVER AND RELEASE AND FULLY UNDERSTAND THAT IS A RELEASE OF LIABILITY. BY AGREEING TO THIS DOCUMENT, I AM WAVERING ANY RIGHT I OR MY SUCCESSORS MIGHT HAVE TO BRING A LEGAL ACTION OR ASSERT A CLAIM AGAINST KELSEY J PATEL FOR NEGLIGENCE OR THAT OF ITS EMPLOYEES, AGENTS, OR CONTRACTORS.
